this week Meridian carpet rundown — we originally gave the wrong designer credit for the silver-grey column, having taken it from another outlet rather than waiting for the press office sheet. Corrected within the hour. Cause: we broke our own rule about second-hand credits under deadline.

three weeks ago Verity Road reshoots — the first version said the additional photography was “two weeks”. It is one. Cause: a source said “a couple of weeks” conversationally and it was written down as a fact.

last month A music item, since removed entirely rather than corrected, reported a festival booking that turned out to be a proposal that was never signed. Cause: one source, published without the single-source disclosure our own rules require. Removing it rather than correcting it was the right call and we would do it again.

earlier this year Fathom Line recast — an early version spelled Delaney Okoro’s name incorrectly in the headline for about twelve minutes. Trivial, logged anyway, because we said we would log all of them.
